By purchasing a ticket you are agreeing and acknowledging the below statements.

Maximum cap of 50 students. 

If we fill up quickly, then we will add a second bus and we can open it up to another 50 students. But we will see how the early registrations go first.

Consent to Participate

I, the undersigned parent or legal guardian, give permission for my child named above to participate in the Sunday School retreat organized by Prophet Elias Greek Orthodox Church which includes transportation by bus from the church to the monastery listed above and return on the same day.

I understand that this retreat includes group travel, walking on monastery grounds, participation in services, meals, and other supervised activities appropriate to the program.

Transportation Acknowledgement

I understand that transportation will be provided by bus and that reasonable care will be taken to ensure the safety of all participants. I acknowledge that transportation and group activities involve inherent risks, including but not limited to accidents, injury, illness, or unforeseen events.

Assumption of Risk & Release of Liability

In consideration of my child’s participation, I voluntarily assume all risks associated with this retreat and transportation.

I hereby release, waive, and discharge Prophet Elias Greek Orthodox Church, its clergy, employees, volunteers, teachers, chaperones, and affiliated organizations from any and all liability, claims, or demands arising from my child’s participation in this retreat, including transportation to and from the monastery, except in cases of gross negligence or willful misconduct.

Medical Authorization

In the event of an emergency, I authorize the retreat leaders or chaperones to seek medical treatment for my child as deemed necessary. I accept responsibility for any medical expenses incurred.

Code of Conduct Acknowledgement

I understand that my child is expected to follow the instructions of clergy, teachers, and chaperones, and to behave respectfully toward others, the monastery, and church property. I acknowledge that serious misconduct may result in my child being sent home at my expense, if applicable.

Photo / Media Consent

I give permission for my child to be photographed or recorded during the retreat for church-related use (bulletins, website, social media).
